![]() ORDER OF COURT granting1 Motion for Leave to Proceed in forma pauperis. The Clerk of Court is directed to open this case and file the complaint. The filing fee is $350.00 and Plaintiff has been granted leave to prosecute in forma pauperis. Accordingly, the Superintendent/Warden of the institution where this inmate is incarcerated is directed to remit from the prisoner's account, in monthly installments, the full $350.00 fee. IT IS HEREBY ORDERED that as soon as funds are available in the inmate's prison account, the Superintendent/Warden shall submit an initial partial payment of twenty percent (20%) of the greater of 1) the average monthly deposits to the inmate's account for the past six months, or 2) the average monthly balance in the inmate's prison account for the past six months. IT IS ORDERED that any and all communication from the Plaintiff to the Court shall be in the form of a pleading or motion that shall be filed with the Clerk of Court at the following address: U.S. District Court, U.S. Courthouse, 208 Penn Traffic Building, 319 Washington Street, Johnstown, PA 15901. IT IS FURTHER ORDERED that Plaintiff must keep the Court advised of Plaintiff's current address at all times throughout this litigation, and as more fully stated in said Order of Court.
