A blank copy of the in forma pauperis motion form will be mailed to Plaintiff along with this Order. Plaintiff is also advised that he is responsible for service of the Complaint upon the named Defendants as outlined in Federal Rule of Civil Procedure 4(c). Therefore, IT IS HEREBY ORDERED that until such time that Plaintiff cures the deficiency, this matter is DISMISSED and the Clerk of Court shall mark this case CLOSED.
