ORDER: 1) Gallman's4 Motion for Leave to Proceed in forma pauperis is GRANTED. pla to pay the $350. filing fee by: (a) payment of the initial filing fee is DEFERRED, (b) the clerk shall open an account in Gallman's name and shall complete a Form EDKY 525 and serve on the warden at the institution, (c) each month Gallman's custodian shall send the Clerk of the Court apayment in an amount equal to 20% of his income for the precedingmonth out of his inmate trust fund account, but only if the amount in theaccount exceeds $10.00. The custodian shall continue such monthlypayments until the entire $350.00 filing fee is paid; 2) Gallman's6 Motion to Appoint Counsel is DENIED; 3) the Clerk shall mark the assigned case number on a EDKY Form 520 and send to Gallman; 4) w/i 30 days Gallman must must file anamended complaint that complies with the requirements using the form provided. Gallman may also include an attachment not exceeding 6 pages; 5) Gallman must take these steps w/i 30 days or the case will be dismissed without prejudice for failure to comply with the Court's Order.
