ORDER - the Court DIRECTS Farley to submit (1) an application for leave to file her complaint that complies with all requirements in the June 2015 prefiling order, (2) a notice indicating whether Farley intends to pay the filing fee or move to proceed IFP if the Court grants her leave to file a complaint, and (3) an amended complaint that does not include any federal causes of action that the Court dismissed on the merits in prior cases. Farley must file these documents within twenty-one (21) days of this Order. The Court, therefore, GRANTS IN PART Farley's motion to file a complaint, (ECF No.1 ), and DEEMS Farley's complaint CONDITIONALLY FILED pending her compliance with this Order and the June 2015 prefiling order and the Court's review of her submissions. The Court WARNS Farley that failure to comply with any of the requirements in this Order or the 2015 prefiling order may result in the dismissal of her case.
